Pediatrics is a field of medicine concerned with the health and medical treatment of infants, children, and adolescents from birth to the age of eighteen. Pediatrics is from two Greek words (pais = kid) and (iatros = croaker or healer) and means “healer of children.” Pediatrics is a relatively new medical specialty that emerged in the mid-nineteenth century. Abraham Jacobi, recognized as the “Father of Pediatrics,” lived from 1830 until 1919. A pediatrician is a person who offers medical treatment to children who are acutely or chronically unwell, as well as preventative health services to children who are healthy. In both sickness and health, a pediatrician is responsible for the physical, mental, and emotional well-being of the children in their care (Mandal, 2019).

This week clinical experience was good, getting to see how my preceptor interacts with the patients and their parents was a learning experience for me. One thing I did like about my preceptor was how she took her time with each patient, she did not rush the parents and every question they had she answered. For the patients who were already in their teens she talks to them directly and educated them on life while the parents were in the room, she included both the parent and child in each education. It was a great learning point for me.

My patient this week was a 4 y.o boy who was brought into the clinic by his mother with chief complain of coughing for the past 4 weeks. The mom stated she had brought the boy in Oct 5 at that time he had had cough symptoms for 2 weeks and when the doctor assesses the patient, he had acute otitis media and was given antibiotics for 10 days. The mother stated she had given the medication as prescribed, and the boy continues with the cough but less ear pain. This week the doctor prescribes prednisone 7ml for 5 days to help with the cough and aerosol treatment as needed. Told the mother to follow up in one week if the cough continues or worsens. To me the hardest part of clinicals this week was watching the children come in sick and being so young unable to express exactly what they were feeling. Seeing them cry was also hard, but it was also fun to play with the patients to try and help them to smile under theses conditions.

Acute otitis media (AOM) is a middle ear infection that is the second most prevalent pediatric emergency room diagnosis, behind upper respiratory infections. Acute otitis media can strike at any age, although it is most prevalent in children between 6 to 24 months. Approximately 80% of all children will have otitis media at some point in their lives, and between 80% and 90% of all children will develop otitis media with effusion before reaching school age. Clinically, otitis media is diagnosed by combining objective findings on a physical exam (otoscopy) with the patient’s history and current signs and symptoms. To help in the diagnosis of otitis media, diagnostic instruments such as a pneumatic otoscope, tympanometry, and acoustic reflectometry are available. When compared to ordinary otoscopy, pneumatic otoscopy is the most trustworthy and has a greater sensitivity and specificity, however tympanometry and other modalities can help with diagnosis if pneumatic otoscopy is not available (Ashurst, 2019).
